Background: In everyday practice, the physicians are facing with variety of causes of back pain.

Among the most common there can be identified musculo-ligamentous disorders, pain associated with herniated degeneration of intervertebral discs (DID), spondolysis (spondylarthritis) viewed in the fair work of the primary syndrome of back pain as well as diseases grouped under the secondary back pain syndrome, including inflammatory diseases of spine, osteoporosis, neoplastic and infections lesions of spine ,chronic pelvic pain.

Objective: Causes of chronic back pain

Primary dorsalgia:

  • Musculo-Ligamentous Disorders

Musculo-Ligamentous disorders are among the most common causes of back pain and often go unrecognized because of the low awareness of physicians. Most clear pathological musculo-ligamentous apparatus reflecting back myofascial pain syndrome (MFS) is characterized by muscle dysfunction and the formation of local diseases seals in affected muscles. Currently MFS can be divided into primary and secondary. The classification takes into account primary muscle dysfunction and the development of MFS on the background of diseases of the spine and other physical conditions.

The main factors provoking the primary MFS are stretching of muscles, repeated micro facture, within overexertion, hypothermia of muscles, emotional stress.

The main reasons of secondary MFS

  • Osteoarthritis (cervical, lumbar ,spondylosis, coxarthrosis, arthrosis of shoulder joint)

  • Structural anatomical changes (scoliosis, torsion of the pelvis, leg length difference)

  • Rheumatoid arthritis

  • Sjogren syndrome

  • Damage to the rotator cuff (collision syndrome), frozen syndrome

  • Dysfunction of the temporomandibular joint

  • Visceral pain (non-stenocardial chest pain, pain in diseases of liver, kidney and urinary tract)

The main precipitating factors of primary MFS

Reasons Comments

  • Muscle strain occurs when the unprepared movement, a bad jump,

twist, etc.

  • Repeated micro traumas micro trauma of muscles most often arise in the

performance of professional stereotyped movements

Overload or after prolong use of untrained muscles.

  • After overstrain it occurs when a long stay in antiphysiological pose (work

in an uncomfortable position of the computer,

professional activity.

  • Hypothermia muscles low temperature promotes muscle spasms

  • Emotional stress accompanied by muscles tension can therefore activate

on trigger print. Muscle spasm may be in a state after the

termination of exposure of stress

  • Acute trauma (fractures, soft tissue damage, postoperative pain)

  • Hypothyroidism

  • Chronic infection (candidias, parasitic disease)

  • Vitamin B12 deficiency
